Cruise ships represent a unique challenge to public health, often becoming hotbeds of infection due to the high density of passengers. The recent case of the ship “Mv Hondius” has brought attention back to viruses such as norovirus or hantavirus, which spread rapidly in closed environments.
Even though they seem like isolated and easily controllable spaces, these ships function as complex ecosystems where food hygiene and interpersonal contact are the most critical points.
Italian virologist Fabrizio Pregliasco analyzes in an interview for Corriere della Sera the reasons why these “cities “cruise ships” require strict international protocols to prevent viral outbreaks.
A cruise ship seems like an isolated and controlled environment. But from an epidemiological point of view, is it really so?
The risk certainly exists. It is a closed place with a great proximity between people and a number of shared services. For example, it is enough to think about food poisoning. On a ship it can spread easily. It is an environment that always requires great attention from a health point of view.
Why can a ship quickly turn into a contact multiplier?
Because it is like a small floating city. People live in great proximity and extended in time. In ordinary life, we probably meet someone on the subway only once. While on the cruise, he meets him today, tomorrow, the day after tomorrow, in the morning and in the evening. This intensity and frequency of contact inevitably increases the risk.
A lot. Restaurants, theaters, swimming pools, common areas, all of which are places of constant contact. I see above all the problem of food poisoning, which is observed even more often, but there are also respiratory infections.
You often cite food poisoning. Is this one of the main risks?
Yes. Recent events show us several episodes of norovirus. They are situations that follow each other. And then there is the issue of general hygiene. On a ship there may be critical areas related to food supply or food handling.
Even in the case of Hantavirus, could the context of the ship complicate things?
Sure. There is the matter of hygiene and disinfection protocols, as well as very strict food safety procedures. But then, these protocols need to be implemented and really controlled.
Is it difficult to distinguish a simple flu from a more serious infection?
Yes, and this is one of the most complicated aspects. Even this episode was probably underestimated at first. But on the other hand, many diseases initially have non-specific symptoms: physical weakness, gastrointestinal disorders, fever. So common symptoms.
Cruises have another characteristic: passengers coming from all over the world. How much does this item weigh?
